PROGRAM ELIGIBILITY CRITERIA

• Health Science Graduates:
  • MBBS, BDS, BAMS, BHMS
  • BUMS, BPTh, BPO, BOTh
  • BASLP, PBBSc (Nursing)
  • B.Sc. Nursing, B.Sc. (Paramedical Technology) or equivalent degree from a recognized university
• Note: Students completing internships are eligible to apply but must provide an Internship Completion Certificate at the time of admission.

DOCUMENTS REQUIRED

1. Nationality Certificate / Valid Passport / Domicile Certificate / Birth Certificate with Indian Nationality.
2. Caste Certificate (If Applicable).
3. Caste Validity Certificate (If Applicable).
4. Non-Creamy Layer Certificate for VJ/NT/OBC/SEBC categories; valid up to 31/03/2027 (If Applicable).
5. Gazette for change in name (If Applicable).
6. Self-Educational Gap Certificate affidavit (If Applicable), certified by Executive Magistrate/Notary.
7. Physically Handicapped Certificate (If Applicable).
8. Self-declaration form for self-attestation.
9. Undertaking regarding registration in the voter list.
10. SSC / 10th Certificate.
11. HSC / 12th Certificate.
12. First to final year mark sheets of the qualifying degree.
13. EWS Certificate valid for financial year 2026-27 (If Applicable).

Note: Selected candidates must submit relevant original documents along with two sets of self-attested photocopies at the time of admission. All category reservations as per State Government rules.
